Outpatient Care for Seniors—Taking Life One Day at a Time For Seniors 65+

Outpatient care is offered Monday through Friday from 9 a.m. to 3 p.m.

The goal of the Geriatric Partial Hospitalization Program is to restore individuals to their highest level of functioning by fostering independence, improving relationships with family and friends and providing patients with a foundation for continued progress.

Daily programming focuses on goal-oriented therapies and consists of the following:

• Complete psychiatric evaluation and assessment.
  
• Individualized treatment plans.
  
• Five therapy groups each day.
  
• Daily assessments and vital sign monitoring.
  
• Transportation (in select areas).
  
• Lunch and snack.
  
• Weekly consultation with a psychiatrist.
  
• Comfortable treatment environment.

Our multidisciplinary team is lead by the referring psychiatrist, but also involves the patient's family physician. Other team members include recreational therapists, specially trained RNs, social workers, and dietitians.
